Machine Readable Indian Passports

In accordance with the International Civil Aviation Organization guidelines, Government of India has decided to introduce machine readable passports for Indian Nationals. The Procedural details of the scheme are as under:

  1. Applicants residing in Panama, Costa Rica and Nicaragua applying for the new Indian Passport have to apply online at website: bulletPassport form and form for miscellaneous services on passport is also available on this website.
  2. Fill PERSONAL PARTICULARS FORMbullet& Affidavitbullet. Take a print out of the application form filled online and P.P. form & Affidavit.
  3. Copy of the application & P.P. form, the relevant application fees and the original passport are required to be deposited in the Embassy.
  4. The online form filled by an applicant will then be transmitted electronically to India by the Embassy for processing and printing of the passports based on Pre or Post PV report.
  5. Passport Issuing Authority in India will ensure dispatch of the new passport within 10 days thereafter. Please click link for stepwise guidelines for filling the application form. These may be perused so that there is no mistake in submission of application which could cause delay in processing. Procedure for obtaining Miscellaneous Passport & Consular Services Applicants for miscellaneous Passport and Consular services are required to fill out appropriate form and submit their passports along with photocopies of their resident card or any other proof of their lawful residence in the jurisdiction of the Embassy.

The practice of endorsement of child's name on either parent's passport has since been discontinued and a new passport is issued to a (newborn) child initially for five-year duration. Photocopy of birth certificate, parents passports (containing spouse name), should accompany the application.

It is very important to note that a birth should be registered, in case a child is to acquire Indian citizenship, within 7 years, if both parents are holding Indian nationality (by birth) (at the time of birth and thereafter), and within one year if one of the parent is not an Indian citizen or one of the parent is Indian (by descent).


Duplicate Passport (IN LIEU OF LOST /DAMAGED PASSPORT) (For application form click here

Applications for duplicate passports are normally not accepted by mail except with prior approval of the Consular Officer. The applicant may be required to come in person.

Please submit your application form with the following documents:

1. Copy of Police Report with English translation duly notarized(for lost passport cases)

2. Six photographs (front pose with light background/dimension 3.5cm x 3.5 cm).

3. Six copies of Personal Verification form (click here)bullet

4. In case of student, letter from school/college is required.

Photocopy of the previous (lost) passport.

6. Photocopy of driver licence or employer's letter (as applicable)

7. Photocopy of identity card/work permit/ stay permit (as applicable)

8. Application in plain paper giving reasons for the loss of the passport.

Misrepresentation of facts may entail denial of passport facilities. Applications received with incomplete details will remain unprocessed. A duplicate passport may not be issued for the second loss/damage of passport. Habitual losers of passport will be subject to enquiry. For change of name, applicant has to submit following documents along with application:

* Court order or notarized affidavit.

* Advertisement clipping of notice in the city of applicants present address.

* Advertisement clipping of notice in the city of applicants permanent address in India.

* Women applicants applying for change of maiden name are required to furnish, along with the application form and spouse   passport, either of the following documents:

1. Marriage certificate

2. Sworn affidavit duly notarized.


i) Ordinary Passport – 36 pages (Valid for 10 years. Also applicable for age-group 15-18)

iii) Ordinary Passport – 36 pages for minors for five years validity (below 18 years)


iv) Ordinary Passport – 36 pages in lieu of lost, damaged or stolen passport


v) Ordinary Passport – 60 pages in lieu of lost, damaged or stolen passport

vii) Misc. Passport Services (PCC or Surrender Certificate or any other miscellaneous services based on Passport)


(The above fee includes charge of USD 2.00 w.e.f.01.09.2017  towards Indian Community Welfare Fund(ICWF) on issue of each document as per Government instructions).

Passport Service: Cancellation of Indian Passports /obtaining Surrender Certificates

All naturalized citizen’s of Indian descent in Panama, Costa Rica and Nicaragua, who have already acquired foreign citizenship voluntarily in the past shall cease to be Indian citizens upon their acquiring foreign citizenship. However, such persons are required to renounce their Indian citizenship and to obtain surrender certificate for their Indian passports, whether valid or expired, so that the passport is not misused. If you have a ‘passport cancelled on acquisition of foreign nationality’ stamp on your passport, there is no requirement of acquiring “surrender certificate”.

For those persons of Indian origin who have already acquired foreign citizenship till 31 May 2010, the fee is US$27. For those, who acquire foreign citizenship on or after 1 June 2010, the fee is US$132.

Attestation of Documents:

Documents issued by Indian Authorities
Birth, Death, Marriage and Educational Certificates etc. issued by the Indian authorities in India can also be attested by the Embassy provided the same have been authenticated by the designated officer of the State Government and validated by the Consular Section of the Ministry of External Affairs (MEA), Patiala House, New Delhi-110001 or any other agency approved by the Ministry of External Affairs. All local  documents originated  in Panama, Costa Rica and Nicaragua for attestation have first to be duly authenticated/certified by the Ministry of External Relations(Foreign Affairs) of  the respective country.

Issue of Police Clearance Certificate (PCC)

In view of the change in policy relating to issue of PCCs, it has been decided by the Government of India that henceforth Police Clearance Certificate (PCC) to all category of applicants will only be issued after receipt of fresh police verification on the PCC application.   All PCC applicants from Panama, Costa Rica and Nicaragua are advised to fill;

Above forms are required to be deposited in the Embassy with all relevant documents well ahead of time as PCC would only be issued on receipt of due police verification report.

Indian nationals in Cost Rica and Nicaragua may send applications for the above services by courier and arrangements for delivery and collection-with all charges paid by them)with prescribed fee to Embassy of India, Panama. There is additional charge of US$20 towards international bank draft/bankers cheque apart from prescribed fee for all passport and consular services.
